Intensive In-Home Services

 Nu-Start Behavioral Health maintains full compliance with all statutes set forth by the Health Insurance Portability and Accountability Act.


  • Prior to admission the client and or Legal guardian is given a copy of Nu-Start Behavioral Health privacy statement outlining the Agency's procedures for compliance with all statues outlined by HIPAA.


  • Each client receives a copy of the privacy statement upon admission to the program. Each client and relevant legal guardian have the right to review the notice and may request information on how Nu-Start Behavioral Health restricts protected health information about each client and how information is used or disclosed for the purposes of treatment, payment, or health care options.


  • A privacy Notice Consent form must be signed, indicating that the client and legal guardian did receive a copy of the privacy statement and had the opportunity to ask questions and request that changes associated with the restriction of private health care information be made. This signed consent form will be permanently maintained in the client’s record.

Intensive In-Home Services

 Intensive In-home Services

 provides high-intensity (7-10 hours/week), time-limited (12 to 26 weeks) therapeutic services for children and adolescents at imminent risk of out-of-home placement (acute psychiatric hospitalization, residential treatment or juvenile justice system) due to a serious emotional illness that cannot be successfully treated in less intensive settings. Services are provided in the home and community while focusing on short-term stabilization, with the ultimate goal being long-term stability for the individual and family. Mental health counselors are available 24/7 to respond to crises and to teach the client and their family to effectively deal with crisis. In addition to the individual and family counseling component of the program, other services that may be provided include parenting training, behavior management, communication and relationship-building, self-management of moods and behavior and school interventions. Intensive in-home services are intensive, short-term, home based, crisis intervention services offering families in crisis the alternative to remain safely together averting the out-of-home placement of children whenever possible. The intensive in-home services program combines skill-based intervention designed to support and empower the family unit. Treatment will focus on reducing psychiatric symptoms and identifying families' behavioral assets and strengths. Providing family centered services according to the family needs in the child's home; family and individual coaching, communication skills, anger management, conflict resolution, parenting education, supervised visitation, nutrition, life skills, crisis treatment, child development, coordination with other required services, fiscal and home management, case management, discharge planning, one-to-­one supervision, mentoring services, and 24-hour emergency response/on-call support.

Service Description


  • Nu-Start Behavioral Services is a therapeutic counseling program designed to support the educational, behavioral, psychological, social and life skills development needs of adults (males and females) aged 18- 65 who demonstrate a significant decline in overall functioning including activities of daily living, social deficiencies and parental responsibilities. Our clients may be dual diagnosed with substance abuse or intellectual disability. 


  •  Individuals must demonstrate a clinical need for the service arising from a condition due to mental, behavioral, or emotional illness, which results in significant functional impairments in major life activities. Individuals eligible for this service may have a dual diagnosis of either mental illness and intellectual disability or mental illness and substance abuse disorder.


  •  Individuals demonstrate difficulty in establishing or maintaining normal interpersonal relationships to such as degree that they are at risk of hospitalization or homeless. 


  •  Individual’s behaviors and symptoms require repeated interventions by mental health professionals, juvenile justice or social services.


  • Individuals have such cognitive difficulties that they are unable to recognize personal dangers that impact them as well as their family members. 


  • Require help in basic living skills, such as maintaining personal hygiene, preparing food and maintaining adequate nutrition, or managing finances to such a degree that health or safety is jeopardized.


  • Client must be willing to participate in services. Client must be between the ages of 18 -65 years old.

GOALS AND SERVICE INCLUDES

Initial Assessment and Preliminary Service Planning

 Each client referred for Mental Health Support services will be subject to an initial assessment to identify their needs, goals, strengths and weaknesses. This initial assessment shall be completed by the Program Director or Clinical Director. The Preliminary Service plan shall be developed as a starting directive for the client’s services. The Preliminary Service plan shall be updated and revised after the initial 30-days of services so that the plan will represent an accurate depiction of the client’s functionality. All service plans will comprise goals and strategies for the client’s advancement and will be developed in collaboration with the client.  

Crisis and Mental Health Support

Any client referred for services who has not completed a psychological evaluation in over 1 year will be scheduled for such with their consent. The mental health counselor will coordinate scheduling, transportation and accompany the client to the appointment. The mental health counselor will participate in a meeting with the client and psychologist in order for the client to understand the results of the evaluation. Any necessary changes/updates to the client’s service plan will be completed at that time. Additionally if the client requires any other diagnostic testing such as educational testing, hearing/sight evaluations, nutritional consultation, physical health screenings, etc. the program will coordinate such appointments in the same manner as previously mentioned.

Coordination of Psychological & Diagnostic Testing

Any client referred for services who has not completed a psychological evaluation in over 1 year will be scheduled for such with their consent. The mental health counselor will coordinate scheduling, transportation and accompany the client to the appointment. The mental health counselor will participate in a meeting with the client and psychologist in order for the client to understand the results of the evaluation. Any necessary changes/updates to the client’s service plan will be completed at that time. Additionally if the client requires any other diagnostic testing such as educational testing, hearing/sight evaluations, nutritional consultation, physical health screenings, etc. the program will coordinate such appointments in the same manner as previously mentioned.

Individualized Service Planning

 The mental health counselor shall create, implement, update and revise the contents of the service plan in collaboration with the client. The ISP shall continue to guide the client’s services by identifying areas of weakness and strengths and proposing goals and strategies for advancement and achievement. The ISP must be reviewed and approved (with signatures) by the client. The ISP shall be updated every 90 days to ensure that the plan remains appropriate and concurrent with the client’s activity and progress.  

Mental Health Stability

 The counselor will help educate the client on their mental health diagnoses, treatment and support in compliance with psychiatric appointments. The counselor will help the client to understand how their mental health impact their overall functioning 

PARENTING COACHING/SKILLS

  Parental skills training maybe limited to helping the client to understand how her/his mental health issues and emotional distress impacts the overall family system 

Substance Abuse Education/Housing Support

Services will provide psycho-education information regarding substance abuse and treatment. The mental health counselor will assist and support the client in obtaining treatment for any substance abuse issues. 


 The mental health counselor will help the client to maintain and obtain housing if applicable. 

INTENSIVE IN HOME SERVICES

Intensive in-home services targets children and adolescents ages five (5) through eighteen (18) who demonstrate a clinical necessity arising from a condition due to mental, substance abuse, behavioral, or emotional illnesses that results in significant functional impairments in major life activities, which places them at risk for removal from their home. Typically, these individuals have been involved or currently involved with multiple agencies such as juvenile justice, social services or mental health.
